Elder Care Management/Guardianship

16 years of Geriatric Care Management experience

Conduct care-planning assessments to identify specific problems to determine abilities, eligibility for assistance and service needs of the client

Determine  and implement the level of care necessary for the client's well being, comfort and safety

Property Management

Screen arrange and monitor Personal Care Aids and in-home help

Assist in finding Home Care or Health Assistants

Oversee the short and long-term care of client in their own residence, nursing homes, ALF's, rehabilitation centers and hospitals

Provide for medication management

Coordinate services provided by medical specialists

Provide counseling, support and care to the client and family members

Review medical, legal and financial issues and offer referrals to geriatric specialists

Provide crisis intervention

Provide client/family counseling and support

Act as a liaison between clients and their families, facilitate telephone contact and visitations, and alert family members of any problems
